Maine's BCBS licensee is Anthem Blue Cross and Blue Shield of Maine, a subsidiary of Elevance Health (formerly Anthem, Inc.), the largest for-profit managed health care company in the Blue Cross Blue Shield Association. Anthem has operated in Maine for decades and as of Kaiser Family Foundation 2019 data held approximately 69% of the state's commercial insurance market — the largest market share of any single insurer in Maine by a significant margin.

Maine residents enrolled in an Anthem Blue Cross Blue Shield of Maine fully-insured plan between February 7, 2008 and October 16, 2020 who filed a valid claim by November 5, 2021 are eligible for an individual payout. Distribution of payments began in May 2026.

Maine's health insurance market is dominated by Anthem to a degree that rivals Alaska's Premera and far exceeds the national average for BCBS licensees. The sole BCBS licensee in Maine is Anthem Blue Cross and Blue Shield of Maine, a subsidiary of Elevance Health, Inc. (NYSE: ELV), formerly known as Anthem, Inc. Elevance Health is headquartered in Indianapolis, Indiana and holds BCBS licenses in 14 states. The Maine subsidiary operates out of Portland and serves individual, small group, large group, Medicare Advantage, and Medicaid managed care members throughout the state.

Anthem Blue Cross and Blue Shield of Maine also administers the State of Maine Health Plan for state government employees, though as a government account those members are excluded from the antitrust settlement. A notable recent development in Maine is that Maine Medical Center — the state's largest hospital and part of MaineHealth — ended its in-network agreement with Anthem in 2023, creating a significant network dispute that highlighted the carrier's dominant position in the state. Despite that dispute, Anthem has retained its status as Maine's largest commercial insurer by a wide margin.

Maine Anthem BCBS claimants with valid claims are included in the May 2026 distribution wave that began May 11, 2026. • Maine State Employee Health Insurance Program (Maine State Health Plan) — State of Maine employees covered through the Office of Employee Health, Wellness & Workers' Compensation are enrolled in a government account administered by Anthem. Because the plan sponsor is the State of Maine, these members are excluded from the settlement regardless of the BCBS administrator.
• University of Maine System employee plans — If the University of Maine System operates a self-funded plan as a quasi-government account, individual employees should verify their plan's specific status at bcbssettlement.com. Fully-insured university plans may be eligible if not structured as government accounts.
• Harvard Pilgrim Health Care / Point32Health enrollees — Harvard Pilgrim (now Point32Health) is not a BCBS licensee. Maine residents whose only health coverage was through Harvard Pilgrim during the class period are not eligible for this settlement.
• Maine Community Health Options (MCHO) enrollees — MCHO is a consumer-operated and oriented plan (CO-OP), not a BCBS licensee. MCHO members are not part of the settlement class.
• MaineCare (Medicaid) — Maine's Medicaid program (MaineCare) is a government program and is not eligible for the settlement.
• Federal Employee Program (FEP) — Federal employees and retirees in Maine covered through the Blue Cross Blue Shield FEP are excluded as federal government accounts.
• Medicare Advantage / Medigap plans — Medicare-based plans are outside the commercial insurance class period and are not eligible.
