The Premier Health lawsuit people are searching for is about a data breach, not MyChart tracking. Someone was inside Premier Health’s systems from June 7 to July 12, 2023; patients were not told until July 2025. A class action followed in Montgomery County. As of October 8, 2026 we found no settlement, no claim form and no deadline — and no Premier Health MyChart lawsuit over tracking pixels.

What Happened in the Premier Health Data Breach
Premier Health is the Dayton, Ohio network of three hospitals and two major medical centers. According to its own notice, it became aware of suspicious activity on July 12, 2023. Its investigation found that files were accessed and taken by an unauthorized party at varying times between June 7, 2023 and July 12, 2023.
Depending on the person, the files held names, dates of birth, Social Security numbers, driver’s license and passport numbers, taxpayer ID numbers, digital signatures, login credentials, financial account information, medical information and health insurance information. Premier Health says services were never interrupted and that it found no evidence of misuse.
The public notice came on July 18, 2025 — about two years after discovery. The notice gave no reason for the gap beyond saying the file review had recently concluded. That delay is at the center of the Premier Health class action lawsuit.
Case Details
The case we found is Monnin v. Premier Health Partners, Case No. 2025 CV 04598, in the Montgomery County Court of Common Pleas in Ohio — a state court, not a federal one. A court-listing service shows it was filed on August 19, 2025 as a class action, with claims for breach of contract and quasi-contract and negligent data security. The lawsuit alleges Premier Health failed to protect the data and waited more than 700 days to notify patients, in violation of Ohio law. Premier Health has not admitted any of this.
We could not open the court’s own docket, so we cannot confirm later rulings. Other law firms announced “investigations” in July 2025; an investigation is an ad for clients, not a lawsuit.
Status: One State-Court Class Action, No Settlement
Tracking: We found no Premier Health MyChart lawsuit over Meta Pixel, Google Analytics or other trackers on its website or patient portal. Many other health systems have faced those cases — see Wellstar or SSM Health — but no such case against Premier Health has surfaced.
Earlier incident: Federal records list a separate October 2, 2020 email phishing report affecting 254,786 people. That federal investigation is closed.

Who Could Be Affected
The federal breach portal lists 10,833 individuals for the 2023 incident. Premier Health itself says it does not disclose numbers. It says it finished mailing notices on August 12, 2025, to people for whom it had an address.
Your letter is the test. If you got a Premier Health notice in summer 2025, keep it: it lists which data was in your files and is your proof that you were notified of the incident. You do not need to sign up anywhere to be included in a class action.
Worried about tracking instead? No tracking case exists against Premier Health today. If you used the Premier Health website or MyChart for appointments, symptoms or bills, our short form asks which one you used, what you did there and when you last used it, so we can tell you if a case ever opens.
What a Case Like This Could Pay
For context only, not a prediction: healthcare data breach settlements usually combine reimbursement of documented losses, a small flat payment for people who document nothing, and credit monitoring. The federal portal lists 10,833 individuals for the incident, which is small, so any fund may be small too. Our data breach settlement calculator shows how comparable deals have paid out, and the MyChart settlement calculator covers tracking cases at other hospitals.

Three Things to Watch For
A breach of Social Security and login data, with no claim form, is an easy story for imposters:
“Premier Health settlement” claim pages
There is no Premier Health settlement and no claim website as of October 8, 2026. A page asking for your Social Security number to “claim your share” is collecting data, not filing a claim.
Calls or texts about your MyChart login
Login credentials were among the exposed data. Premier Health recommends that, if login credentials were involved, affected people change relevant passwords and use multifactor authentication where possible. Never give a password to anyone who calls or texts; reset it yourself at the real MyChart site.
Callbacks to a number you were not given
The incident line Premier Health published is (855) 507-7876. If a letter or caller gives a different number, look up Premier Health’s contact details yourself before you call back.
Premier Health Lawsuit — Questions People Actually Ask
Is there a Premier Health class action lawsuit?
Yes, over the data breach. Monnin v. Premier Health Partners, No. 2025 CV 04598, was filed August 19, 2025 in the Montgomery County Court of Common Pleas. As of October 8, 2026 we found no settlement in it.
Is there a Premier Health MyChart lawsuit over tracking?
Not that we could find, as of October 8, 2026. Many hospitals have been sued over Meta Pixel and similar trackers, but we found no case against Premier Health. Its portal is MyChart by Epic, at mychart.premierhealthpartners.org.
Is there a Premier Health settlement I can claim?
No. There is no settlement, no fund, no administrator, no claim form and no deadline that we could find. If one is approved, class members would be notified by a court-appointed administrator.
How many people were affected by the Premier Health data breach?
The federal breach portal lists 10,833 individuals. Premier Health says it does not disclose numbers as a matter of policy.
Why did it take two years to tell patients?
Premier Health has not explained it beyond saying its file review recently concluded. The lawsuit alleges the delay was more than 700 days and violated Ohio law; Premier Health has not admitted that.
Is the Premier Health breach letter real?
Premier Health mailed real notices in summer 2025 with free credit monitoring. Check that any letter matches the incident (activity between June 7 and July 12, 2023) and call only the published line, (855) 507-7876, or a number you look up yourself.
Do I have to do anything to be included?
No. If a class is certified or a settlement is reached, you are included unless you opt out. Keep your notice letter as proof.
What should I do right now?
Enroll in the free credit monitoring if you have not. Freeze your credit at all three bureaus. Change your MyChart password and turn on two-step login. Watch insurance statements for care you never got.
Separate from this case: were you injured in the last 2 years?
Class-action payouts are fixed amounts through an administrator. A personal injury claim is a different case — and often worth far more. Free estimate, no obligation.
