MDLIVE is a telehealth platform owned by Evernorth Health. The mdlive lawsuit alleges that tracking technology on the MDLIVE website disclosed users’ sensitive information and private communications to Google and Meta without consent.

Who Can Claim
You are in the settlement class if you are an MDLIVE user who accessed patient.mdlive.com and did anything on the portal after signing in between May 30, 2023 and August 11, 2025.
To be paid, you must also attest on the claim form that you had an active Facebook and/or Google account during that period. You do not upload documents, but the attestation is made under penalty of perjury.
The online form asks for the Claimant ID printed on your notice. A paper claim form can be printed from the official site and mailed to Kroll at PO Box 225391, New York, NY 10150-5391.
What the MDLIVE Settlement Pays
The $3.5 million figure is a cap, not a pot. Under the notice, the defendants pay valid claims, notice and administration costs, attorneys’ fees, and incentive awards — all together, up to $3,500,000. Class counsel may ask for up to one-third of the cap in fees plus up to $15,000 in expenses, and each of the two named plaintiffs may ask for up to $5,000. If valid claims plus those costs exceed the cap, every payment is cut pro rata.
Payment is by check, PayPal or Venmo. Checks become void 90 days after they are issued, so cash yours promptly.

Look-alike claim sites
The only claim site is EHISettlement.com. A page with a similar name asking for your MDLIVE login, insurance ID or Social Security number is not the claim form — the real claim form does not ask for a login, insurance ID or Social Security number; it asks you to attest that you used the portal and had a Facebook and/or Google account.
“Pay a fee to release your $15”
Nothing is paid before final approval, and no step costs money. Any message asking for a fee, a gift card or bank login to speed up your MDLIVE payment is fraud.
Fake “confirm your health records” requests
