What Happened and What the Lawsuit Said
Medical Associates of Brevard (MAB) runs six clinics around Melbourne, Florida. The settlement agreement says that on January 24, 2025, MAB discovered that cybercriminals had gained access to its network and taken files with patients’ private information.
When MAB announced the data breach later that year, the reported data included names, dates of birth, Social Security numbers, driver’s license or state ID numbers, medical treatment and health insurance information, and financial account information. MAB said it identified the affected people on or about July 7, 2025. A copy of its letter was filed with the Maine Attorney General on September 5, 2025. The HHS breach portal lists 246,711 people.
A ransomware group claimed the attack. MAB has not verified that claim. The lawsuit alleges MAB failed to protect patient data. MAB denies any wrongdoing, and no court decided who was right.
Case details
This is a Florida state-court case, not a federal one. The plaintiff first sued in Brevard County on March 17, 2025. After mediation, the parties signed a settlement dated August 28, 2025. They agreed Broward County was the proper venue and refiled there in late August 2025.
The case is Beloli v. Medical Associates of Brevard, Case No. CACE-25-012989, in the 17th Judicial Circuit Court in and for Broward County, Florida. ClassAction.org reports preliminary approval on September 7, 2025. The settlement administrator is Simpluris, Inc.
Status: Settled, Claim Window Closed
One more thing we noticed: on October 18, 2026, the settlement website (MedAssociatesBrevardDataBreach.com) did not load at all. If you filed a claim and are waiting, contact the administrator at (844) 496-1203 or MAB Data Incident Settlement, c/o Settlement Administrator, P.O. Box 25226, Santa Ana, CA 92799.
Breach notices from other practices, such as Family Medical Associates of Raleigh, Vernon & Waldrep OB-Gyn Associates and Radiology Associates of Richmond, are separate cases.
Who Was in the Settlement
The court defined the class as “all living individuals residing in the United States whose Private Information may have been impacted in the Data Incident.” Excluded were MAB and its officers, government entities, and the judge and court staff.
Your mail is the test. Class members got a court notice with a unique ID and PIN for the online claim form. Many had already received MAB’s breach letter offering Experian credit monitoring. The class covers everyone whose information may have been impacted, so if you are unsure whether you were included, ask the administrator at (844) 496-1203.
If you were in the class and did not opt out by December 1, 2025, you are bound by the settlement. That is true whether or not you filed a claim. You gave up the right to sue MAB separately over this breach.
What the Settlement Pays
Documented losses: up to $1,500 per person for out-of-pocket losses between January 24, 2025 and December 1, 2025. You needed proof such as receipts or bank statements. Covered losses include fraud losses, credit report or credit freeze fees, ID replacement and postage.
Medical data monitoring: one year of CyEx Medical Shield Pro, which comes with $1 million of medical identity theft insurance.
No flat cash payment. People who documented no losses could only claim the monitoring.
Class counsel asked for $300,000 in fees and costs, plus a $3,000 service award for the plaintiff. Some sites call the $300,000 a settlement amount. It is not: it is the fee request. ClaimDepot reports payments go out about 105 days after final approval, if there are no appeals.

Three Things to Watch For
With the claim window closed and the official site offline, these are the traps most likely to reach MAB patients:
“Late claim” or look-alike settlement sites
The deadline was December 1, 2025. A site that offers to take a new MAB claim, or asks for your Social Security number to “release” a payment, is not the administrator.
“Pay a fee to receive your $1,500”
The $1,500 was a maximum, paid only for documented losses. The defendant pays the costs. No real administrator charges a processing fee or asks for gift cards before sending money.
Calls and texts about your doctor’s office
The stolen files reportedly held names, birth dates and insurance details, so a scammer can sound convincing. Hang up. Call the number on your own MAB letter, or the administrator at (844) 496-1203.
Medical Associates of Brevard Breach — Questions People Actually Ask
Short answers to the questions people ask most about this case.
Which company got the data breach?
Medical Associates of Brevard, a physician group with six clinics in Brevard County, Florida: Indian Harbor Beach, Melbourne, Palm Bay, Rockledge, Satellite Beach and West Melbourne. The breach was discovered in January 2025. When Medical Associates of Brevard announced the data breach, it reported 246,711 affected people to HHS.
How much compensation will I get for a data breach?
In this settlement, it depends on what you documented. The cap was $1,500 for documented out-of-pocket losses, plus one year of medical identity monitoring. People without documented losses could only get the monitoring. Claims closed December 1, 2025. Our data breach settlement calculator shows what comparable cases pay.
Can I still file a claim?
No. The claim, opt-out and objection deadlines all passed on December 1, 2025. If you filed on time and want to check your status, contact Simpluris at (844) 496-1203.
Can you sue a hospital for a data breach?
Patients can and do sue healthcare providers over breaches. This case shows how it works: a patient sued MAB in March 2025, and the case settled in August 2025 without any finding of wrongdoing. Class members who did not opt out by December 1, 2025 gave up their right to sue MAB separately over this incident.
Is there a class action lawsuit against Change Healthcare for data breach?
That is a different breach at a different company. This MAB settlement does not cover it. See our Change Healthcare data breach lawsuit page.
Has the settlement been approved and paid?
The final approval hearing was set for December 16, 2025. As of October 18, 2026, we could not find the approval order or a confirmed payment date. ClaimDepot reports payments go out about 105 days after final approval, if there are no appeals.
What data was exposed?
The court notice says the files may have held names, Social Security numbers and health information. Reporting on MAB’s breach letter adds dates of birth, driver’s license or state ID numbers, health insurance information and financial account information. Your own letter shows which of these applied to you.
What is worth doing right now?
Freeze your credit at all three bureaus. It is free and reversible. Check your health insurer’s explanation-of-benefits statements for care you never received. If you enrolled in the Experian monitoring MAB offered (12 to 24 months), note when it ends. Keep your MAB letter and any settlement notice.
